Issue
I just added a tool to the toolbar, but now that I have closed the Customize dialog, I do not see it.
Details
This can occur if you accidentally changed the Modality selection in the Customize dialog (possibly with the scroll wheel). For example, if you are viewing a CT and save a tool on the CR context toolbar, you do not see that tool when you close the Customize dialog.
Another possibility is that the added tool is not available under the active license or the current permissions. You are not prevented from adding tools you are not authorized to use.
Solution
Try the following to solve the problem:
Add the tool again, this time being sure not to change the Modality value.
If you have been assigned another license type, change your active license and see if the tool is available.
If permissions are the problem, contact your PACS Administrator about getting access to the tool you have added.
